You’ll appreciate how much a catastrophic injury can change lives and the importance of creating safe, effective, and enabling environments for injured people and their families. The role: As Clinical Case Manager, you will join our network of specialist clinicians to support some of the most vulnerable children, young people and adults following life changing injuries such as acquired Brain Injury, Spinal Cord Injuries, complex orthopaedic injuries, amputation and limb loss, psychological injuries and more. Your responsibilities will include: • managing a case load of clients through their rehabilitation. • leading on assessment in the home and their rehabilitation and goal setting. • signposting and advocating for what they need, and assessing what needs to be in place for the people you support to live a fulfilled life, whether that be access • services, reintroduction into their communities and social lives, accessing work or meaningful activity and so on. • Managing all administration in relation to cases, including sending instructions to third parties and writing and reviewing reports. As an associate case manager you will work with people who have suffered catastrophic injury to: • Assess their needs, understand them pre-injury and identify their needs post-injury • Put plans in place based on these needs for a successful rehabilitation with clear, manageable goals • Implement the plans alongside the client (including the parent or carer of children and young people) • Coordinate the relevant care and support whether that be accessing statutory services, therapy and rehabilitation, financial support as well as educational needs, vocational support, hobbies and lifestyle choices • Monitor progress and respond to changing needs.
