About the opportunity As a Lead Physiotherapist, you will be responsible for: Assessing new and existing residents mobility and rehabilitation needs, including identification of the most appropriate walking aids. Identification of residents at risk of falls and implementation of preventative measures. In partnership with the nursing team, writing and reviewing mobility and handling care plans and risk assessments. Rehabilitation, treatment and maintenance programmes for residents, including setting up and reviewing of individual goals. Strengthening & Balancing Programmes, and soft tissue therapy/massage. Exercise classes for residents, including falls prevention, mobility exercises and resident wellbeing. Increased confidence and mobility of residents, leading to reduced care needs and reliance on care team members. Regular liaison and handovers with care and clinical team members. To develop discharge plans when required G eneral Duties Deliver group exercise classes: a core element of this role is providing tailored exercise classes to our residents to encourage them to stay active, reduce the risks of falls, maintain their strength and balance. We understand the importance of independence and encourage residents to maintain their mobility. The classes are designed to suit the abilities of the residents, including seated classes for less mobile residents. To contribute to regular goal planning and ensure that information is shared/ communicated as required. To write accurate and professional reports to timescales for third parties in respect to musculoskeletal assessment, diagnosis, management and expected recovery. Work within a multidisciplinary team to deliver high standards of care to the residents living at KYN Hurlingham. Work collaboratively with therapy colleagues in the delivery and evaluation of specialised therapy interventions for residents at KYN Hurlingham. Be an autonomous practitioner, responsible for the provision of specialised assessment, treatment, education and interventions. Responsibility for managing own caseload of clients with varied and complex long-term conditions and complex needs. Work as a Physiotherapist to help the service prevent avoidable hospital admissions. Work within and adhere to the Health & Care Professions Council (HCPC) standards. Be professionally and legally responsible for all aspects of his/her own work in accordance with the HCPC standards, and KYN Policies & procedures. Support senior and junior clinicians in service delivery and be actively involved in service developments with the team Keep up to date with latest evidence-based practice underpinning the area of work. Be able to utilise a wide range of assessment and treatment skills to formulise specialised programmes for residents. Communicate in a manner that is consistent with legislation, policies and procedures. Undergo an annual appraisal programme, in line with KYN policies and procedures, ensuring the highest possible standards of care through commitment to CPD. Attend mandatory training and adhere to all KYN policies and procedures. Clinical Apply clinical reasoning skills to the assessment, planning and implementation of the therapy process in consultation with the resident, carer and multi-disciplinary team. Monitor and evaluate treatment plans, maintain accurate records for all residents and manage any identified risks appropriately. Manage a complex and varied caseload effectively and efficiently with regards to clinical priorities and use of time. Demonstrate treatment and reasoning skills required for therapy assessment and treatment, using appropriate therapeutic models, approaches and handling techniques. Use evidence-based practice, alongside own knowledge, skills and experience: and in line with legislation, policies and procedures; Gain valid informed consent, prior to the delivery of interventions, which must be documented and have the ability to work within a legal framework with residents who lack capacity to consent to treatment. Involve the individual in all aspects of their treatment programme, respecting their dignity, wishes and beliefs throughout all interventions. Utilise a range of communication skills to instruct, inform and negotiate in order to achieve active resident participation, a cohesive approach to treatment and successful case management. Communicate effectively both verbally and in writing with other disciplines, departments and organisations e.g., MDT meetings, case conferences, discharge planning and report writing. To contribute to regular goal planning and ensure that information is shared/ communicated as required. Take an active part in the delivery and implementation of the clinical governance agenda for the service and the department. To monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention. To discuss services and treatment options to residents. To ensure that residents are involved in the planning and prioritisation of their rehabilitation plans wherever possible. To attend case review meetings and provide physiotherapy input as required. To demonstrate high level of clinical effectiveness by use of evidence based practice. Work as a lone practitioner with support available from a senior management if required. Managerial Support the development of others and assist in their application of knowledge and skills in practice. Support the training of qualified and unqualified staff so that asks can be delegated appropriately. Support the supervision of junior staff and students within the service. Identify and manage risks involved in work activities and processes for self, colleagues and residents. Embrace change within the service as improvements are implemented. Assist in monitoring the effective implementation of systems of work in the department.
