About SPOTWELLBEING CIC: SPOTWELLBEING CIC is a community interest company offering a range of services. We set up the service in 2019 and now run one-to-one sessions and groups across Suffolk. The service is run by health professionals: Occupational Therapist, Physiotherapist and technical instructor with support from our new administration and business support team member. Our mission is to support people to have a good quality of life, doing activities that are meaningful to them through holistic, personalized partnership working. We have a one-to-one service for people over 50 who require support to improve their wellbeing. People can expect to be contacted by a team member and seen at home for sessions. The length of service will depend on the individual and what they are working towards. We are holistic and will work with the person on whatever we are able to, to improve their wellbeing. Our Health and wellbeing Groups are 90-minute sessions, once a week for six weeks. 2 of our team members lead 45 minutes of exercise, practice relaxation techniques, have a talk / discussion on a topic related to ageing well and then the group can socialize and get to know each other. The groups run across Suffolk. We also have a sustainability project linked to these groups – supporting communities to continue them independently. Job Purpose To co-facilitate SPOTWELLBEING CIC Health and Wellbeing groups across Suffolk. To develop the groups as required to meet the needs of the communities we work with. To assist in the provision of the SPOTWELLBEING CIC personalized and holistic one-to-one service across Suffolk. The post holder will work supportively and collaboratively with the Clinical Team Lead in ensuring the effective management of Spot in all respects, environmental, financial and clinical in so far as this relates to direct and indirect clinical practice. The post holder will monitor clinical practice on a day to day basis, providing supervision and guidance for the team as required. The post holder will implement reviews and changes in practice in collaboration with the Clinical Team Lead. The post holder will be able to and deliver high quality evidence based Occupational Therapy/Physiotherapy services. The post holder work alongside the Clinical Team Lead to plan the delivery of Occupational Therapy/Physiotherapy services. The post holder will work with clients with complex needs using evidence based / client centred principles to assess, plan, implement and evaluate interventions. This will include: · Assist in the provision of one-to-one sessions: · To work with the team to meet people who are referred to the service at their home/ in the community. To get to know them and build up a partnership – learn about what is important to them and how they want to improve their wellbeing. You will follow the SPOTWELLBEING CIC model. · To seek to understand each person and their complexities, strengths, and areas they want to improve. · You will complete assessments for one-to-one support plans for people that have been referred to the service. · To work with other services and local providers as benefits each individual person. · To make any referrals / reports as required by the person to support their wellbeing. · Highlight/ discuss referrals/support plans with other team members. · You will also be required to: · Maintain accurate clinical records to a high standard. · Use outcome measures with clients to inform impact data for the service and individual outcome measures if appropriate. · Applying for funding writing bids · To participate in MDT meetings and team meetings. · To keep your own up-to-date CPD · To engage in regular supervision organised by yourself on regular basis every 3 months. · Assist in the running of health and wellbeing groups: · Setting up spot health and wellbeing groups helping to organise the educational talks/updating the talks when necessary. · Be able to talk in front of people and take a class instructing exercises and mindfulness talks · Good knowledge of falls prevention/ blood pressure issues / frailty / mental health awareness · Making drinks and chatting with attendees Essential criteria for applicants: · HCPC registered PT or OT · Degree in Occupational therapy or Physiotherapy · Proficient skills in IT, math’s and English. · Experience in working in the community with complex patients · Excellent listening skills. · Car driver with vehicle essential · Enhanced DBS check required for position.
